How to Submit a Nomination | KSHA-ASHF Clinical Achievement Award
This award recognizes a speech-language pathologist or audiologist in Kentucky for significant contributions to the advancement of clinical knowledge and practice. The recipient will be named the Kentucky Clinical Achievement Award Winner and will also serve as Kentucky’s nominee for the ASHF Louis M. DiCarlo Award for Outstanding Recent Clinical Achievement, whose national recipient receives a $2,000 grant and recognition at the ASHA Convention. Accomplishments must have occurred within the past six years.
Nominees are evaluated based on their advancement of knowledge in clinical practice, demonstrated by significant recent accomplishments in speech-language pathology or audiology. Examples of eligible contributions include:
- Achievement with a single client or group of clients
- Development of clinical service programming
- Program design or implementation
- Contributions to local/state organization or government initiatives
- Clinical testing, research, or administrative leadership
Nomination Procedures
The Honors Committee and ASHF Board rely exclusively on submitted materials, so nominations must be complete and clearly written.
Submission Requirements (limit: 10 single-spaced pages, including support letters):
- Nominee’s full name, title, place of employment, mailing address, and phone number
- Description of the clinical achievement, including when and where it occurred
- Explanation of the significance and impact of the achievement
- Statement on how the nominee’s work has advanced knowledge in clinical practice
- Description of the nominee’s specific role in the project or achievement
